In the hospital and unable to use a urinal!

My son was placed in a full length leg cast the third day in the hospital.  He continued to experience excruciating pain which prevented changes in position.

We discussed a condom catheter, but, they: 1) frequently cause irritating rashes and excoriation if the male is allergic to latex or even the adhesive 2) generally leak 3)  further degrade the person using them  4)  rarely stay in place.  So , we used the diaper to urinate into and then threw it away.

Day 4 or 5 my son was placed in a half body cast with a spica bar.  He continued to have pain, but did tolerate minor position changes.

The spica bar compounded the problem of using a urinal.  I spoke with all of the nursing staff and the Occupational Therapist concerning our problem.  No solution was found.  We searched medical catalogs and I went to every pharmacy in a 30 mile radius without any success in finding any male urinary devices My son in his castthat would meet my sons needs.  THIS JUST WOULD NOT DO!

So, I invented the Male Urinary Drainage System or MUDS.

I’m a “victim” of the common urinal used in hospitals…I was flat on my back after surgery and spilled every where.  I asked them “what would the president use?”  “one of these, or a catheter”, was the response.

I would’ve gladly paid $40 for your solution to not suffer the indignity.  I’m an engineer and thought to invent a solution as well; didn’t know about yours at the time.  Even got one moving.  However, my investigation into the matter showed that the hospital put more like a $0.40 price tag on my dignity!  I’m sure you already knew that…

During the design process, the inventor took into consideration numerous handicaps.  Decreased or limited vision makes it difficult to use a urinal.  Placement or the urinal can be managed, but,  the problem occurs with the attempt to place the urinal with urine in it onto a surface after use.  It causes frequent spills.

Tremors are uncontrollable shaking of the hands.  They can be mild to severe and limiting to varying degrees based on their intensity.

Being wheelchair bound doesn’t mean that you are incontinent.  It may mean, simply, that you are unable to stand for any significant period of time in order to urinate into the toilet.

What Makes a Male Urinal so Difficult to use

Problem #1 – a Male urination problemsrequires that a man have enough length to point his penis into the urinal.  As men age, pubic fat frequently envelops the penis and causes less available length to point into a urinal.

 Problem #2 – he also has to maintain the appropriate angle of the urinal to prevent urine from spilling back onto him. This is very difficult, especially if the man has any sort or restrictive appliance or cast.  Urine will not run up hill and if the appropriate angle is not maintained – there goes the urine all over the man and any surrounding area.

How to Empty and Clean the MUDS

Routine cleaning of the MUDS will prevent bacteria and mold growth and extend the usage of the unit.

Emptying the MUDS:  Unscrew the tubing from the drainage container and place the cap on the container.  Measure the urine using the markings on the side.   Remove the cap and pour the urine into the toilet carefully to avoid splashing.  Reconnect the drainage container to the tubing at the bed or chair side and place the container in the desired location.

The container will hold at least 2500ml in the upright position.  It is recommended that at least once every 24 hours the unit be rinsed out with water or a 50/50 vinegar solution to prevent crystals from building up in the tubing or unit and for odor control.  The container should be emptied at least every 12 hours and as needed.  Do not let the urine fill above the 3000 ml mark or it will inhibit the urine drainage into the container and make emptying the container difficult. 

Any Male urination problemsrequires routine cleaning to preserve sanitation and maintain the expected life of the unit.

Helpful Hints using the MUDS while lying on your back

        Males with urine trouble, such as inability to use a urinal due to the use of only one hand or restrictions in movement due to spinal injuries, can be more independent in urination with the use of the MUDS.  Here are some helpful hints to promote efficient use of the MUDS.

     LYING FLAT ON YOUR BACK:   (Remember it is a gravity system) Slightly bend knee and place tubing under leg of choice. Place over penis with textured side up. Apply gentle pressure to groin area and press unit down gently into the scrotum. This will provide the appropriate angle for gravity drainage. The soft edge will not scratch or irritate. Do not leave tubing under leg for extended periods. It could cause pressure sores.

LYING FLAT ON YOUR BACK WHEN SPINAL PRECAUTIONS MUST BE MAINTAINED:  Use the 2 and 3 ft sections of tubing to form a 5 ft connection (remove the threaded connector from one end of each section of tubing, then use the connector provided to splice the 2 sections together) Hang the drainage container toward the foot of the bed. To use: run the tubing between the legs and use as above. If the bed is capable, slight reverse trendelenberg will greatly improve gravity drainage. This will allow the user to urinate independently and minimize to eliminate any leakage. Again, ensure that the tubing is against the bed when not in use to prevent falls.

How to setup a MUDS

The key to optimizing a MUDS is in the setup.  Males withMale urination problemssuch tremors or just plain diffuculty using a urinal will benefit from the MUDS.   

   Choose the appropriate place to attach the holder. Consider whether the user is left or right handed, which side would be most appropriate for safety, or give the most privacy. The holder seats over the bedrail. Wrap the Velcro strap around the holder. Select the appropriate length of tubing. In general, if the user is going to be lying in the bed, he will need the 3 ft. section. If he will be sitting on the side of the bed or sitting in a chair, the 2 ft section will probably be more appropriate. Remember that it is a gravity system and select the length of tubing that will provide the most direct drainage to the container ffor best results. The containers’ position can be adjusted also to promote drainage. Position for the most direct drainage without loops if possible. Remember to eep the tubing and the container out of the walk path to prevent tripping.
     Connect one end of the selected tubing to the MUDS device and one end to the container. Place the device in the holder and the system is ready for use.

The shape and slope of the MUDS unit is designed to assist in appropriate angle during use to promote drainage of urine.  The urine does not stay in the main unit, but drains into a container which will hold up to 2500ml of urine.

The solution (part III)

The Doctor anticipated 8-10 weeks in the body cast.  I began to utilize tubing, bottles, and other strange supplies in an attempt to develop a system that would work.  I finally came up with a solution.  My very crude, homemade Male urination problemsworked.

We had a few small leaks due to my inability to make a sealed system, but after the first time I showed my son how to use the device, he was able to urinate by himself.  Without Mom, Dad, or Nana disposing of his diaper.  Although he was still cranky and difficult, it was not compounded by his embarrassment over the bodily function of urinating.

My son successfully used my urinary system for the next 16-18 weeks.  At home he was able to urinate in the middle of the night or day without depending on anyone to help him.  We took him to the Doctors office in his reclining wheelchair.  He could push himself into the bathroom, take his device out of the bag, and urinate directly into the toilet.

AND SO, MY INVENTION BEGAN

My son was placed in a full length leg cast the third day in the hospital.  He continued to experience excruciating pain which prevented changes in position. 

We discussed a condom catheter, but, they: 1) frequently cause irritating rashes and excoriation if the male is allergic to latex or even the adhesive 2) generally leak 3)  further degrade the person using them  4)  rarely stay in place.  So , we used the diaper to urinate into and then threw it away.

Day 4 or 5 my son was placed in a half body cast with a spica bar.  He continued to have pain, but did tolerate minor position changes.

The spica bar compounded the problem of using a urinal.  I spoke with all of the nursing staff and the Occupational Therapist concerning our problem.  No solution was found.  We searched medical catalogs and I went to every pharmacy in a 30 mile radius without any success in finding any Male urination problemsthat would meet my sons needs.  THIS JUST WOULD NOT DO!

Why I needed to invent the MUDS

The invention of the Male Urinary Drainage System was spawned from a need that the medical community could not provide.  My son sustained a double comminuted fracture of the femur when he was 11 years old.  He was hospitalized for 8 days.

The first 2 days he spent in traction lying flat of his back.  Even minor position changes caused excruciating pain.  The first time we attempted to use a bedside urinal was a catastrophe.   The urinal could not be positioned at the appropriate angle and not pour urine back out onto the bed.  Turning to his side or sitting up was out of the question.  The only solution the nursing staff had was a condom catheter or letting him urinate into a diaper and then dispose of it.

His dignity and pride were shattered.  Between the pain, and his inability to perform his own toileting needs without assistance, he was angry and difficult.  This why I needed to invent the Male urination problems.

Excessive urination vs frequency and urgency

Excessive urination, frequency, and urgency, are descriptive symptoms of a dysfunction in our urinary habits.  

The term Excessive urination refers to urinating an abnormally large amount of urine over a given period of time.    The average amount of daily urine for an adult is 1.5 liters or just over 1 and 1/2 quarts.  Excessive urination is considered over 2.5 liters per day or just over 2 and 1/2 quarts. 

 Frequent urination  refers to how often your body requires you to empty your bladder.   On average we empty our bladders about ever 5 hours give or take an hour. 

   Urgency is when you have the overwhelming urge to urinate regardless of whether or not your bladder needs to be emptied.  It is overwhelming and often anxiety producing due to the sudden onset of the urge to urinate.

Just thought someone out there might appreciate this story.  However, I’m quite sure that my daughter won’t.  Speaking of urinary problems!

My family went on a camping trip when my daughter was 4 years old.  You know, a nice campground with male and female bathrooms with shower stalls.  I woke up early and decided to go fishing.  My husband stayed at the campsite with our daughter – just sleeping the morning away.  She woke up and needed to go to the bathroom.  Of course, her Daddy took her to  the camp restrooms and proceeded to instruct her to go to the bathroom and that he was going into the male restroom.  That if she finished before he did, she was to wait right outside the door.  Soooo, Daddy hurriedly used the bathroom and waited patiently outside the female bathroom/shower area for his little girl.  After a few minutes when Daddy’s little girl did not come out of the bathroom, he called out “Michele, are you O.K.?”.  Michele answered “Yes Daddy, I am O.K..  So, Daddy waited,…. and waited some more.  He repeated, “Michele are you O.K.”.  and Michele states “I need somebody to wipe my butt”.  Daddy said “Michele, I can’t come in the bathroom, you will have to wipe your own butt”.  Michele began crying and wailing that she needed somebody to wipe her butt.  Daddy finally asked her, “Michele, is there anybody in there?” and Michele answered “Yes Daddy”.  So Daddy states again that he can’t come in the bathroom, that she will have to wipe her own butt, and the wailing began all over again.  Daddy noticed that no one had come or gone from the female restroom and again asked Michele, “Is there anybody in there with you?”.  and Michele proceeded to again answer, “Yes, Daddy” and then she added “Me”.  So, Daddy did go into the female restroom to wipe Micheles’ butt and after that Michele had to learn to wipe her own butt.

Problems with using a male urinal

Using urinal?  Male urinals are difficult, at best, to use.  Frequently male anatomy changes with age.  The penis sometimes envelops into pubic fat, reducing length and making it hard to point the penis into a urinal.  Frequent misses are common and spilling after use is easy to do.  Then, the urinal has urine in it and you need to use it again.  Of course, there is no one to empty it for you or they are not timely enough and you attempt to use it again with urine already in it.  Now a difficulty task becomes even harder, resulting in spills.

I have found that the best way to clean male plastic urinals is to use vinegar.

I rinse with a 50/50 vinegar and water solution as needed to clean and control odors.

Male frequent urination – preventing falls in the elderly male

Male urination problemssets up the environment for an increased risk of falls.  When hurriedly trying to get to the bathroom to urinate, the risk increases. 

It is important to encourage elderly males to ambulate, but hurried bathroom trips are not the appropriate encouragement to get up out of the chair.  It increases the risk of falls and causes anxiety.

Reasons to avoid catheterization in males who are continent.

One of the many reasons to avoid inserting a foley catheter in a continent male is the risk of trauma.  Insertion of a foley catheter is painful and has the associated risk of causing injury to the urethra or prostate. 

There is a high probability of bleeding when the catheter is traumatically removed with the bulb intact.  For example:  If the patient is getting out of bed, ambulating, or misjudges the length of the tubing when hurriedly getting out of bed to go to the bathroom for a bowel movement the catheter could accidentally be tugged and traumatically pulled out.

When injury occurs, treatment may include:  reinsertion of the foley, bladder irrigation, blood transfusion, or a increased hospital stay with increased acuity of care.  Traumatic injuries related to urethral catheters  can lead to shock and potential long term consequences as a result of the injury. 

Try other alternatives to a catheter in continent males before inserting a foley catheter.

MUDS VS a Sheath Urinal

A Male urination problemsis designed to be worn continuously and has its place for incontinent males.  However they are uncomfortable, the tubing frequently kinks limiting drainage into the drainage bag , and they require that the male have enough length to apply it.   They have their place for incontinent males to help decrease the need for other incontinent supplies such as diapers.

Being a female, I have never worn a condom catheter, but I have placed many of them.  You clean the area really well with soap and water and then you dry, and wait for it to dry some more.  Liquid adhesives are sometimes helpful, but can you imaging having adhesive placed there?  Then you are finally ready to place the condom catheter.  You roll it on only to find out that it is too large or too small, and you don’t have the appropriate size.  If you have the right size, you generally get pubic hair under the condom catheter and that must be mighty uncomfortable.  You finally get it placed, and how long does it stay on?

Seriously, condom catheters are not comfortable, but sometimes they are the only alternative for an incontinent male.  If you must place a condom catheter, make sure that it is an appropriate fit – not too snug and not too loose.  If the male has not been circumcised, do not pull the foreskin back.  Leave it in its’ natural position and place the condom catheter over the foreskin, otherwise long term retraction of the foreskin will cause swelling and could even require medical attention to treat.  As I always say, always put the turtle back in its’ shell before applying the condom catheter. 

If you know a continent male who is using a condom catheter just because they can’t use a urinal without spilling, suggest that they try a Male Urinary Drainage System.  It is designed specifically for males who have difficulty using a urinal.

It may be used in virtually any position as long as the tubing and main device are in position to drain by gravity. Place the MUDS unit over the penis. The orientation textured surface is to be up. Gently press against groin area forming a light seal. Urinate into device. When finished urinating, raise device to straighten tubing and allow all urine to drain into the container. Replace the MUDS device into the holder and apply the lid. The SECRET to the MUDS working at it’s best is in the setup. The container needs to be placed to allow for the most direct route of drainage from where the device will be used, limiting loops or sags in the tubing.

Knee replacement

“My father had bilateral knee replacements. He had several complications and set backs and had an extended stay in the hospital. Because of both knees being operated on at the same time, he had a very difficult time using a urinal. It was hard for him to sit-up, turn from side to side or even bend his knees to accommodate a urinal without spilling it in the bed. To position the urinal was also difficult due to the continuous positie motion machines placed on his legs for range of motion.
My Dad was embarrassed and discouraged due to this problem. A foley catheter was not an option at this point in his care and I felt so bad for my Dad. I approached Jacque for help, and she devised the external urinary device from simple materials and it was a miracle. My father was able to use this device without once spilling the urine in the ed and the design was so user friendly that he could actually place the device himself, even laying flat on his back. From that point on, my father had almost an immediate change in attitude and his progress improved. He felt in control agian, no longer helpless.

ABOUT US

M.U.D.S.,L.L.C. was started in 2004 as a L.L.C. with the intent of developing and producing the MUDS system. Jacquelin Vantroostenberghe is a Registered Nurse and the inventor and owner of the MUDS patent. Her son sustained an injury which resulted in a fracture requiring a body cast. He was unable to urinate into a traditional urinal due to the restrictions of the cast and being flat of his back. He was in pain with any movement and embarrassed that he had to have help with urinating.
                                   
It was impossible to place a urinal at the right angle without urine pouring back onto him. Jacque and the occupational therapists searched catalogs, medical supply stores and pharmacies without any success finding a alternative. His only alternative at that time was to urinate into a diaper.

And so, that is what inspired the development of the MUDS. Jacques’ son used a crude version with great success and was independent with all of his urination needs with the exception of needing someone to empty the container.

Necessity, the Mother of all Invention

Hi, my name is Jacque Vantroostenberghe and I am a Mother, Daughter, Registered Nurse, and I am an inventor. 

          As a mother, there is nothing in this world that hurts more than when your child is hurting. My son suffered a terrible accident that resulted in a 3/4 body cast for over 8 weeks. I am fortunate and blessed that I had the knowledge and ability to help him during his recovery. My son was devastated to find himself using a diaper or depending on Mom, NaNa, Aunt Kathy, or his sister to aide with his toileting needs. So, I began developing what I now call, the MUDS. it was a lot of trial and error, but the system worked. My son was completely independent of urination with the exception of needing someone to empty the container. The MUDS became like my purse, we took it everywhere we went. My son was able to use the device in the Doctors office, public restroom, and even in the car. When he was out of the cast, he continued to use the MUDS, especially at night, until he no longer needed crutches.

       As a Registered Nurse, I see every day in my personal, social, and work environment males who need the same thing that my son did. Not just the ability to urinate on their own, but the ability to do it in a sanitary, private, and convenient way. 

      My Dad was on a short trip and got stuck in Dallas traffic. He takes a “water pill” and when the urge calls, he has to go—–right now.  He had to resort to using a plastic grocery bag and hope that it wouldn’t leak.
My father-in-law also takes “water pills”and has breathing difficulties. He tires easily and at times is very shaky–his hands from his medications to help him breathe, and his legs when he is feeling tired. He has to get up in the night to urinate and has almost fallen on multiple occasions.

      This is just a hint of the males out there who need the MUDS to help keep them from falling, promote sanitation, aide in preventing unnecessary catheterizations, and the reason I invented the MUDS in the first place, to promote their self-esterem and preserve their dignity.
